This study aimed to identify direct and indirect nursing care time in a medical and surgical ward. Methods: In this cross - sectional study 44 medical and surgical nurses from East Guilan Hospitals were reviewed. Data gathering instrument was made of three parts: demographic data, time ranging, nursing and non-nursing care. Each nurse was evaluated within 5 alternative shifts. Data analysed using statistical methods in the form of descriptive statistics (frequency tables, mean, standard deviation) ANOVA. Results: The average time spent on direct nursing care 21/5±11/1 and on indirect nursing care 22/1±10 minutes per patient. The origin time in direct and indirect nursing care was different between the wards, hospital, work experience and marriage status. Conclusion: The spending time on direct nursing care for each patient was less than standard. The time spent on direct and indirect nursing care was variable in different wards, depends on organization of the hospital. Diagnosis of bacterial vaginosis via clinical Amsel criteria remains widely competent. Objective: The aim of the study was to evaluate the effect of tinidazole tablets via Amsel criteria in Patients diagnosed Bacterial Vaginosis. Methods: In This randomized clinical duble-blind trial study, 110 non-pregnant women aged between 15-45 years old were referred to specialist clinic in Kangan city in 2012. Following their diagnosis confirmed bacterial vaginosis via Amsels criteria they divided in two groups for treatment. 52 woman received 2g tinidazole tablet one per day for 2 days and 48 women treated with 500mg metronidazole table twice per day for 7 days. Patients were evaluating two weeks following their treatment. For statistical analysis, descriptive statistics, t-test, McNamara, x2 and Fisher's exact test were performed. Results: After two week consumption of tinidazole tablet 19.2% of cases had abnormal vaginal discharge, 15.4% had positive whiff test, 21.2% had vaginal PH>4.5 and in 9.6% of cases, clue cell was more than 20% in vaginal discharge (p<0.001). After two week consumption of metronidazole tablet 16.7% of cases had abnormal vaginal discharge, 16.7% had positive whiff test, 29.4% had vaginal PH>4.5 and in 8.3% of cases, clue cell was more than 20% in vaginal discharge (p<0.001). Conclusion: This study showed that oral tinidazole and metronidazole were efficient on all four Amsel criteria. Prone positioning amongst premature infants could improve their oxygenation compared to supine position. Side lying position leads to developed support between premature infants, however there is few studies about effects of this position on premature infants 'oxygenation. Objective: This study was conducted to hemoglobin oxygen saturation (SPO2) in the side lying position compare to supine position in infants under mechanical ventilation. Methods: In a clinical trial 32 preterm infants hospitalized under mechanical ventilation in Neonatal Intensive Care Unit (NICU) on university hospitals of Isfahan city. they were processing in randomized method, 120 minutes in supine position and then turn to side lying position for another 120 minutes .Their SPO2 were monitored by pulse ox meter and the results recorded every one minute. Data analysis was done by ANOVA, one way ANOVA and post hoc LSD tests. Results: Means of SPO2 in time of 0 were 95.6±2.4 and 95.5±2.4, and in time 0f 120 were 95±5.5 and 94.8±2.6 in supine versus lateral position. There were no significant differences between mean of SPO2 during 120 minute in each position .The differences of mean of SPO2 between the supine and lateral position were not significant except at the time of 45 and 105 minute. Conclusion: Side lying position was well tolerated by preterm infants under mechanical ventilation. Objective: This study aims to determine the effectiveness of management teaching within two learning process techniques using tutorials and small group teaching for Nursing and Midwifery student. Methods: In this quasi-experimental study, all students (Term 3 and Term 6 Bachelor Students of nursing and midwifery, from Guilan University of Medical Sciences) approved management tutorial courses were studied (93 individual's census). During 8 theory sessions, two ways of teaching and discussion in small groups (4 sessions) were designed. The results of the quiz scores (for teaching time in lectures and teach in small groups) were compared. Data using descriptive statistics (frequency distribution and mean) and inferential statistics (t-test). Results: The results suggested that teaching Average Quizzes mean score was significantly higher in group discussion method then teaching as a lecture (p<0.001).It showed nursing students (48±3.8), students of Bachelor Students(69±87.7), students of Midwifery (71±7.7), Bachelor Students nursing student (73±72.7). Conclusion: As a result, average scores for medical students in small group teaching were higher. Objective: This study has planned to determine ADHD symptoms among preschool children based on rate of recurrence by parents report. Methods: this cross-sectional study were carried out for 8 months on 300 children at age of 3-6 years old who referred to children’s clinic in 17 shahrivar hospital in Rasht. Data collection were carried out using Pavuluri,s questionnaire method which contained two section, in the first section, demographic information of the patients and family , in the second section, children’s symptoms were recorded. Date analysis performed via descriptive and analytic, x2 and t-student. Results: In the present study the Age’s distribution were between 3-6 years old which 60.3% of the symptoms were found among 5 year old children. The boys (58.3%) were highest participant. Frequency of ADHD was 67.3%. The most symptoms reported by parents were producing high sound in the play field and jump in middle of conversation and intervention in other children play field. Statistical communication was significant, between age (P=0.004) and father education level (p=0.001) and between birth number (p=0.001), sex (p=0.001) and family’s money rate (p=0.05) with frequency ADHD. Communication wasn’t between age and mother education, job, parent relative, breast feeding during. Objective: The objective of this study was to determine the effect of preoperative cares with holistic approach prior to surgical procedure on the patient’s anxiety and fulfillment. Methods: In this quasi experimental study 78 patient facing prostate surgeries were selected through convenience sampling method during 2010 at Birjand Emam Reza Hospital and allocated to two groups of 39 patients. The involvements consist of determine patients’ needs physically, socially, psychologically, religious and cultural such as prevent hospital infection and providing holistic cares prior operation using 5 questions. The control group received routine cares. Data were gathered with the Amsterdam Anxiety Scale and one researcher-developed patients’ satisfaction questionnaire then were analyzed by descriptive statistics, Chi-square, independent and paired t-tests. Results: Experimental and control groups in relation to age, education and place of residence were similar. After intervention the total mean anxiety score of experimental group (14.30±2.66) was lower than control (15.38±1.51) group (p<0.031). The satisfaction of experimental group (91.15±5.35) was higher than control (80.69±11.89) group (p<0.0001). Conclusion: Preoperative holistic cares for reduction of anxiety and improve satisfaction between patients prior to prostate surgery was efficient. Among this the health of nurses who are important part of country health system is under greater danger than other social groups due to a variety of reasons. Objective: This study aims to determine the level of health and its related factors in nurses employed in medical-educational hospitals in Rasht. Methods: In this cross sectional study, 374 employed nurses in 8 Medical- Educational Centers affiliated to Guilan University of Medical Sciences were selected by stratified sampling and evaluated using Bio- data questionnaire, and Goldberg General Health (GHQ) Questionnaire. Data were analyzed by using descriptive (mean, median, standard deviation) and inferential statistics (Mann-Whitney U, Chi square). Results: Findings showed that 111 (29.7%) nurses were healthy while 263 of them (70.3%) were unhealthy. The highest mean score of unhealthy nurses was related to social performance domain (11.35 ± 3.13) and the lowest mean was related to depression area (2.18 ± 2.86). The results also indicated a significant statistical relationship between the variables of general health with marital status (p=0.057), care of elderly and handicapped child (p<0.001), partnership (p=0.010), job status (p=0.013), organizational post (p=0.04), second job (p<0.001), and job interests (p<0.001) in nurses. Objective: This study aimed to compare some factors associated with urinary incontinence in women with and without urinary incontinence. Methods: This descriptive case-control study was conducted on all women who had rectocel and cystocel surgery from 1997 to 2007 in Al-Zahra and Poursina Hospital. In this study all cases for cystocel and rectocel (480cases) were considered as case group and twice as many (960 cases) normal women were selected as control groupand compared after matcting variable of age, number of deliveries, blood pressure, diabetes, menopusal status and hormon replacement thrapy. Dara were analyzed using descriptive statistical methods (mean and percent) and analytical tesrs such as T-test and Chi-squre. P-value under 0.05 was considered significant. Results: The mean age of patients with urinary incontinence was 43.10 ± 11.89 years. Average number of parity in case group was higher than the control group (3.81 ± 1.26 vs 1.98 ±0.75) and statistically significant differences were observed between the two groups (p=0.0001 ). Patients in both groups had no significant differences considering diabetes, hypertension, menopause and hormon replacement therapy. But there was statistically significant differences in type of delivery between the two groups (p=0.0001). Conclusion: It was observed that urinary incontinence in women with higher parity and vaginal delivery was higher than the control group. Providing religious care to those in needs is patient's right and it's an important religious duty. Objective: This study aimed to determine the condition of religious care provided to hospitalized patients in selected educational centers in Rasht city. Methods: This is a descriptive-analytic study. One hundred patients hospitalized in medical-surgical units in one of the selected hospitals in Rasht city in 2011were chosen by convenience sampling method. Data were collected through a researcher made questionnaire to survey the religious care provided to hospitalized patients that included 20 questions in domains of sex conformity, condition of patient privacy, praying condition, visiting and confidentiality and condition of praying facilities in hospital. Collected data were analyzed by using descriptive statistics and Friedman test. Results Most samples (72%) were older than 45 years. Most were female (55.6%) and housewife (55.4%). 85% had education under diploma. Patients had the most sex conformity with nurses and least conformity with the doctors but it was statistically non-significant (p<0.097). Patient privacy compliance and condition of praying was unfavorable and there was no significant difference between different units (p=0.009). Conclusion: Overall findings indicated that care provided was unsatisfactory in most domains surveyed. Objective: The aim of this study is to determine the adequacy of prenatal care and its relationship with neonatal outcomes. Methods: This analytic historical cohort study was performed on 205 pregnant women referring to health centers in Rasht city. Data gathering instrument consisted of a four part questionnaire covering demographic data, data related to current pregnancy, data related to neonate and adequacy of prenatal care utilization index. Care was classified in four groups: intensive, adequate, moderate and inadequate. Data were analyzed using by descriptive statistics and, Chi-square and Spearman's correlation coefficients. Results: Most cases of preterm birth, fetal death, low birth weight, neonatal jaundice was in the group with inadequate prenatal care (p<0.05). There was no significant difference in four groups as regard to age, employment, education, body mass index and attendance of childbirth preparation classes. There was no significant difference found in four groups between height and head circumference at birth and adequacy of prenatal care. Nurses due to their presence and practice in treatment centers, direct contact with patients and also many stresses are at risk of various types of violence. Objective: The aim of this study was to determine the incidence of violence against nurses working in educational-medical centers in Rasht city. Methods: This is a descriptive-analytical cross sectional study which its population consists of nurses employed in educational-medical centers in Rasht city and 442 nurses were selected by stratified randomize sampling methods. Data collection instrument was extracted from Nachreiner and Rahmany tools used to survey incidence of violence against nurses in the educational-medical centers in Rasht. The findings were analyzed by descriptive and inferential statistics such as T and Chi square tests. Result: Findings showed that 54.1% and 11.1% of nurses experienced verbal and physical violence respectively. Verbal violence in most cases occurred by visitors and physical violence often occurred by patients and in most cases who did not report violence was because it was considered useless. In addition results indicated a significant relationship between verbal violence and level of education (P= 0.02) and physical violence and work condition (P=0.02) and marital status (P=0.02). Objective:, in this study we examined the association between educational status and identity style. Methods: This cross-sectional study is a descriptive - analytical study which was conducted on 371 students of Guilan University of Medical Sciences. The samples were selected using a stratified systematic. The questionnaire was consist of demographic characteristics and standard questionnaire-style identit, respectively. In order to evaluate the educational status, final mark of students were applied. Data were analyzed using parametric statistical tests (mean ± SD) and bread-parametric (Spearman and Mann-whitney and kruskal-wallis). Result: Spearman's test showed that the normative identity style (P<0/03) and commitment (P<0/001) is correlated with the average total students. Conclusion: there was correlation between normative identity style and commitment statistically. . Objective: The aim of current study was to determine dimensions of posttraumatic growth in patients with cancer. Methods: This is cross-sectional descriptive study. Study population included all cancer patients of two main referral hospitals in Tehran and samples consisted of 402 patients who had the entrance criteria were chosen by convenience method. Instruments used included demographic characteristics questionnaire and Posttraumatic Growth Inventory. For data analysis, descriptive and analytical statistical tests (T-test and Variance analysis) were used. Results: Mean age of samples was 46/7 ± 14 and 60% were female, 38/5% had metastatic cancer. The mean Posttraumatic Growth Inventory (PTGI) score of the participants was 68/68 ± 14/68. Among dimensions of post trauma, the dimension of "spiritual changes" and "communication with others" had the highest score (7/63±2/13 and 25/03±4/9 respectively). From different variables age (r= 0/193 P<0/001) had reverse relation with post traumatic growth and significant difference was shown between level of education, (P<0/013), income (P<0/15) and growth after trauma. Study data obtained were analyzed using descriptive statistics and independent tests, Chi-square, Variance analysis and Linear Regression. Results: The mean gestational age in fasting group was 3338±498gr and 3343±339gr in no fasting group. Findings indicated no significant difference between the mean neonate birth weight born to fasting and no fasting pregnant woman (P<0.931). However, how to communicate with children is important and may relate to their parenting style. Parenting styles play an important role in the formation of abnormality and normal personality behavior in children. The use of adaptive styles in children can lead to many negative consequences, including behavioral problems. Objective: The aim of this study was to investigate the relationship between parenting styles and children misbehavior (internalizing and externalizing). Methods: This cross-sectional descriptive study performed with randomly selection on 741 student aged 6 to 12 first grade to fifth grade public and private schools of 1th and 2th region in the city of Rasht in 2012. The information collected tool was Baum rind parenting style questionnaire and teacher reports. The data was analyzed using descriptive and Pearson correlation and multiple linear regression method stepwise. Results: The results showed that parents of subjects with authoritative parenting style were the most (96/5%) with a mean of (SD) 41/26 ± 8/29. Also, 12/1% of children have externalizing misbehavior and 8/9% of them have internalizing problems. Results indicated that authoritative style had negative significant relationship between internalized behavior problems score (p<0.003) and externalizing problems score (p<0.020). There was a significant positive relationship between permissive style and internalized behavior problems score (p<0.006). The results of regression analysis showed that authoritative style is a negative significant predictor with all internalizing and externalizing behavior problems and permissive style is a positive significant predictor of all internalizing behavior problems. Conclusions: Authoritative parenting, with associations of control under high passionate support, creates independence and conjoint relationship between child and parents, which could characterize an important role in prevention of problems behavioral disorders of children. Objective: The present study was conducted to determie the relationship between severity of nurses’ burnout and tolerance and their sense of humor. Methods: This correlational descrirtive study was conducted on 60 employed nurses in hospitals of Ghazvin province and targeted sampling method was used. Measuring instruments used included Sense of Humor Questionnaire (SHQ) and job burnout of Jackson and Maslach and resiliency of Kanner and Davidson(CD-RISC). Data were analyzed using One-way variance analysis test. Results: Study findings suggested that burnout severity was less in nurses with high sense of humour than nurses with low sense of humor (P<0.029). Frequency of job burnout in nurses with high sense of humor was less than nurses with low sense of humor (P<0.0045) and also, resiliency of nurses with high sense of humour was more than nurses with low sense of humor (P<0.0001). For data collection, one part of Gurses performance obstacles questionnaire was used which contained background and demographic information and information about performance obstacles associated with technology and equipment during a shift. Data was gathered through interviews conducted by the researcher. Data analysis was conducted using descriptive statistics. Results: Findings suggest that the major performance obstacles associated with technology and equipment experienced by nurses respectively include lack of equipment (50%), having to use old and poor conditioned equipment (46.9%) and non-isolated beds or rooms assigned to nurses (33.8%). Objective: This study was conducted to determine dietary pattern, breakfast and snack in guidance school students in Ramsar city. Methods: In this cross sectional descriptive analytical study 208 governmental middle schools were selected by cluster sampling. Data were gathered by demographic questionnaire and HFFQ (Harvard’s Food Frequency Questionnaire) . Data were analyzed using descriptive and analytic statistics (chi-square, kruskal –wallis) . Results: Based on study findings, the main food group consumption of most participants (56.2%) were unsatisfactory. 92.8 % of samples ate breakfast at home and 96.2 % had snack in school. Samples always consumed milk products (34.1%) meat (10.6%), egg (13.9%), vegetables (17.4%) and fruits (34.1%). There was significant relationship between sex and frequency of milk (P< 0.02), meat (p<0.00001), egg (p< 0.004) and sours consumption (p< 0.04). Method: This is an experimental study with pre and posttest with control group. Samples included cancer patients being treated in Shafa Hospital in Ahvaz. They were selected by simple random method. At first, all patients completed the general health and life expectation tests. Then, 30 patients who had the lowest scores in these tests were selected. The training of hope therapy was done in 8 sessions each session lasted 90 minutes. After finishing sessions, posttest was administrated to experimental and control group. Results: Data analysis showed a significant difference in mean scores of control and experimental group (p0.01), so the training of hope therapy leads to improving hope to live and general health in experimental group. Data were analyzed using descriptive statistics and independent t-test, ANOVA, Pearson and Spearman correlation and multiple linear regressions. Results: The mean scores between participant for general and specific communication skills were 53/23 and 121/60 respectively. Communication skills level amongst students was at moderate to good. The mean scores of variables such as marital status, communication skills training and experience in patient care did not showed significant (p<0/05).The results showed a positive correlations between age, presence of the clinical environmental conditions and communication skills. Conclusion: The results of present study showed that communication skills of students are at moderate towards good. Part one included standard questionnaire of Cooper Smith Self-esteem Inventory which surveyed 4 domains (general, education, familial and social) and second part was a researcher made questionnaire covering related factors to self esteem including two parts of personal and familial factors (age, gender, height, weight, order of birth, grade level, major, last year’s Great Point Average, religious activities, physical and emotional illness, apparent problems, number of family members, education, occupation and age of parents, family monthly income, place and condition of living). For data analysis descriptive statistics and Kruskal-Wallis and Mann-Whitney U tests were used. Results: Findings indicated that only 37.8% of cases studied had good self-esteem. Objective: This study aimed to determine the effect of dialysate temperature on vital signs, comfort and adequacy of dialysis in hemodialysis patients. Methods: This study is a clinical trial conducted on 30 hemodialysis patients hospitalized in hemodialysis ward of one of Hamedan hospitals. Eligible patients were selected by simple sampling method and then three sessions of interventions were done. Patients were dialyzed at 37º C in first level and 35ºC and 36ºC in the second and third levels during three sessions. Patients’ blood pressure and pulse rate were controlled before and after dialysis and in the first, second and third hour during dialysis. But patients’ body temperature was controlled before and after each session. In the third session, patients’ blood samples were collected before and after dialysis to assess the KT / V. At the end of session, patients were assessed for comfort using visual criteria. For data analysis repeated measurement, variance analysis and Tukey test were used. P value less than 0.05 was considered statistically significant. Results: Research findings showed that although with reduction of dialysate temperature, systolic blood pressure of patients slightly increased, but this increase was not statistically significant. Patients' pulse reduced significantly by decreasing. Patients were more comfortable at lower temperature. And only 6.7% of patients preferred 37C º temperature. Dialysate temperature did not matter to 20% of patients while the rest preferred cool dialysate. Patients’ KT/V showed a significant increase at 35C º temperature compared to 36ºC and 37ºC. Delay in rabies vaccination increases the risk of rabies after animal bites. Objective: This study aimed to determine incidence rate of rabies vaccination delay after dog bite and its related factors in Guilan province. Methods: In this cross-sectional descriptive analytical study conducted over one year period from July 2011 to June 2012 we surveyed all those with at least 16 years of age with dog bite who referred to rabies vaccination centers in Guilan province. A researcher-made questionnaire was used for data collection and data were analyzed by descriptive statistics, Chi square test, independent t test and logistic regression. Results: During research period 1604 cases of dog bites were surveyed of whom 13.8% of cases had at least 24 hour and 5.5% had at least 48 hour delay for receiving rabies vaccine. Having superficial injury (OR: 2.16, CI: 1.34-3.48), incidence in rural area (OR: 1.69, CI: 1.05-2.72) and lack of knowledge about necessity of rabies vaccination after dog bite (OR: 1.45, CI: 1.07-1.97) significantly led to delay of rabies vaccine after dog bite. Methods: This study is a descriptive and correlational study whose statistical population consisted of nurses in health centers in Rasht city, of which 322 patients were selected randomly as samples. Data collection tool included nurses’ professional behavior taken from Wakefield instrument and Research Agency questionnaire and quality of health care for patient safety culture. Data were analyzed using descriptive statistics and Pearson correlation coefficient. Results: Findings indicated from respondents' views, nurses’ professional behavior and patient safety culture in educational health centers in Rasht was at moderate level. The results also showed a significant and direct relation between professional behavior of nurses and patient safety culture (P <0.01, r=0.6). Conclusion: Patient safety culture will improve as nurses’ professional behavior increases. In attention to high prevalence of cardiovascular disease, coronary artery bypass grafting is a common technique for its treatment. Contrary to belief of most patients, this method has the effect of relief and not treatment. Therefore adherence to care instructions after surgery is very important. Several factors can affect the adherence of care instructions. Objective: This study aims to determine the status of adherence and its related factors in patients undergoing coronary artery bypass surgery. Methods: In this cross-sectional study, 217 patients referring to Dr Heshmat educational therapeutic center undergoing coronary artery bypass surgery in more than 6 months ago were selected by systematic random sampling method and studied through telephone interviews using the adherence of care instructions and illness perception questionnaire. For data analysis Chi-square and Fisher's exact test were used and multiple logistic regressions were used to determine the factors associated with adherence. Results: Findings showed that majority of patients had appropriate adherence to smoke cessation (88%) and physical activity (60.8%), but most study cases had inappropriate adherence to diet (88.5%). Regression results showed that predictive factors for adherence to smoke cessation included sex (P=0.003) and illness perception (P=0.04), and in regard to adherence of physical activity also sex(P<0.0001) and illness perception(P=0.057), and adherence to diet only sex(P=0.048) were indicated as predictors. Conclusion: According to the study results, adherence to care instructions is not complete. Since studies have shown that personality has a role in determining individuals’ health behavior ,thus it is possible to change one’s health behaviors like self medication through suitable interventions. Objective: Present study aims to determine the relationship between personality characteristics and self use of antibiotics among students. Methods: In this co relational descriptive study, 405 students of Qom universities entered the study by available sampling method and were asked to complete both questionnaires of Neuroticism Extraversion Openness Inventory and demographic information. Data were analyzed through‌‌ descriptive and inferential statistics such as t-test, Mann Whitney and chi-square. Results: Study participants consisted of 255 female and 150 male students with an average age of 21.76 ± 4.39 years. 53.8% of students stated using antibiotics in last three months and of which 49.8% obtained antibiotics by physician prescription, and the rest (50.2%) without a prescription. The most common cause of antibiotic use was symptoms of cold and rhinorhea and Amoxicillin was the most used antibiotic. No feeling of need to refer to physician during illness was listed as the most common reason for self-medication with antibiotics. Study results indicated that participants had moderate level of neuroticism, extraversion, agreeableness and conscientiousness. The mean score of personality dimensions showed no significant difference in two groups of self-medication users and non-users (P>0.05). Conclusion: Considering the high prevalence of self-medication among students, it is necessary to increase awareness and promote development of students' attitudes and performance through proper interventions. Methods: This descriptive cross - sectional study was conducted on 426 children aging 6-7 years old, who were selected by stratified randomized method from two districts of Rasht city from all children who referred to National program of children health measurement. Data collection instrument consisted of a researcher made questionnaire to assess predictors of overweight and obesity and sample’s demographic characteristics. Data were analyzed using t-test and ANOVA and Pearson correlation test. Results: The results showed that 8.6% of boys and 14 percent of children are overweight and 19% of boys and 15.3% of girls are obese. There was a positive relationship between children’s overweight and obesity with family history of obesity (p<0.001), maternal BMI (p<0.02), amount of food eaten in comparison with same age children (p<0.001) and the amount of physical activity for children (p=0.02). Conclusions: Four variables of family history of obesity, maternal body mass index, and parents’ view on food taken compared with other same age children and hours of physical activity were shown as predictors of overweight and obesity in children entering elementary school. Tracheal suctioning which is done with the aim of secretion removal can be used in conjunction with physiotherapy for effective airway clearance and improving gas exchange. Objective: The aim of study was to determine the effects of rib cage compression before suctioning on respiratory parameters in mechanically ventilated patients. Methods: In this crossover clinical trial study fifty mechanically ventilated patients were randomly divided to two groups. Each patient received two interventions of endotracheal suctioning with and without rib-cage compression in expiration time with a minimum of 3-hour interval between the two interventions and ten times with three respiratory cycle intervals. Oxygen saturation and dynamic compliance were measured before, 5 and 25 minutes after and data were analyzed using paired t-tests and Greenhouse Geisser and Sphericity. Results: Average cumulative dynamic compliance a minute before, 5 and 25 minutes after intervention with compression was 30.06, 32.14 and 30.93 and 30.42, respectively and without compression, 32.26 and 32.81. Arterial oxygen saturation a minute before, 5 and 25 minutes after the intervention was 97.67, 98.09 and 97.94 respectively in suction with compression and 98.12, 98.27 and 98.17 without compression which were almost similar in the 3 step evaluation and there was no statistically significant difference. Results: There was a significant relationship between some indicators of quality effectiveness such as patient satisfaction, number of inspected complaints and improved procedure within implementations of ISO quality system (p<0.05). Methods: This is a cross sectional descriptive study which surveyed the level of self-care of 326 patients with rheumatoid arthritis referring to Rheumatology clinic of Razi educational center in Rasht. Data were collected using Health Assessment Questionnaire in order to determine the disability rate and Self-Care Inventory in order to determine the Self-Care ability rate through interview. Data were analyzed by descriptive and analytical statistical tests (Kruskal Wallis, Mann Whitney and Multiple linear regressions). rResults: The mean self-care ability of patients was 57.65±2.32 which was at a good level. Sex (P<0.0016)), age (P<0.001) marital status (P<0.001), education (P<0.001), jobs (P<0.001), income (P<0.004), duration of illness (P<0.000) and co morbidities (P<0.001) were significantly associated with self-care ability. This surgery has various complications. Electrolyte disorders are among common important complications among these patients. Objective: purpose of this study was to determine the level of electrolyte disorders and related factors in patients  after coronary artery bypass grafting  surgery hospitalized in cardiac care units. Methods: In this descriptive cross–sectional study, 288 Coronary Artery By Pass Graft (CABG) patients admitted to educational hospital were selected by gradual sampling. Data were collected by a three part researcher made questionnaire covering  demographics,  past medical history and levels of sodium and potassium through patients’ medical records. Statistical analyses were performed using appropriate tests (chi square test, fisher exact test and logistic regression). Rیافته ها: نتایج نشان داد که سبک فرزندپروری مقتدرانه هم با نمره کل مشکلات رفتاری درونی سازی (003/RResults: Findings showed that 16.7% of patients undergoing coronary artery bypass grafting were hyponatremia, 4.9% hypernatremia, 14.2% hypokalemia and 3.8% hyperkalemia. The results also indicated that there was a significant relationship between the factors such as body mass index, history of drug use, the number of vessels involved, the number of grafts, the length of time connected to mechanical ventilation, duration of artificial heart-lung machine use, aortic clamping time, and amount of intraoperative hypothermia, hemoglobin and hematocrit and postoperative electrolyte disorders (P<0.05). According to Logestic regression model, these factors were not associated to electrolyte abnormalities. Objective: Therefore the aim of this study was to comparing the situation of the foot skin in diabetic patients with and without foot ulcers. Methods: In this cross - sectional study of 355 diabetic patients referred to Razi Hospital in 2013 were randomly selected based on a questionnaire consisted of four parts: demographic profiles, clinical characteristics, skin situations and diabetic foot, by a trained nurse were studied. History of the disease based on Charlson Co morbidity Index and foot ulcers by Wagner ulcer classification system were evaluated. Patients were divided into two groups of foot ulcer and non-ulcer and each of the variables were compared between these two groups. We used Chi square test and logistic regression analysis to compare some variables. P-value less than 0.05 was considered significant. Results: Based on these findings, 16.1% had foot ulcers. Dry skin with no sweating, pallor, fisher, edema, impaired growing toe nails in the group with ulcer was (89.47%, 42.1%, 26.31%, 21.05%, and 64.91%) and in the group without ulcer was (73.48%, 35.9%, 11.74%, 12.08%, and 48.65%). There is significant correlation between foot ulcers and dry skin (p=0.01), fissure (p=0.004) and impaired growth toenail (p=0.02).Risk of foot ulcers in people with dry skin 3.07 times, the people of Fisher 2.68 times in people with impaired growth and toenails 1.95 times higher. Objective: The present study have been conducted to determine the effect of Benson relaxation on anxiety in patients waiting for electrophysiological diagnostic and therapeutic interventions. Methods: This is a clinical trial study on 72 patients waiting for electrophysiological interventions, admitted in Heshmat Hospital - Rasht.  Samples selected through convenience sampling method, and were allocated to intervention (usual care and relaxation) and control (usual care only) groups through Random Block method.  Data were collected using the 2- part questionnaire include personal - clinical data and hospital anxiety and depression scale (only anxiety part) through interview and document records information.  Data were gathered in 2 stages, first before 60-15 minutes and second 5-3 hours before electrophysiology intervention (personal-clinical characteristics and level of anxiety).  Intervention group encouraged to listen to audio instructions Benson relaxation for 10 minutes using headphones, and then exercise for at least 3 times between two steps of study.  Data were analyzed by X2, paired t- test and independent t-test rResults: The results showed that the mean and standard deviation of anxiety score in the intervention group after the intervention has been much lower than before (3/11 ± 3/43) compared to (7/91 ± 6/70).  Paired t-test between the mean scores before and after relaxation in the intervention (P <0.  0001) and control group (P <0.  0001) was significant.  Finally, independent t-test showed a significant difference (p<0.  0001) between the mean and standard deviation of anxiety score in the second step in two groups, which indicate the effect of relaxation on anxiety patients. Data were gathered by a researcher made questionnaire, consisting of 48 items in 7 domains of clinical governance. Questionnaire validity was done by face and content determination style and its reliability was established through Test-retest method and Spearman test (r=0.73).  Data were analyzed by descriptive statistics. rResults: Clinical audit and personnel management had the most negative effect in clinical governance administration from participants’ viewpoint. From all questions "inattention to personnel job satisfaction" in "personnel management" domain had the most negative effect. Objective: The aim of study was to determine the professional ethics standards compliance in nursing practice from nurses and patients prospective in 2012. Methods: This is a descriptive comparative study that was conducted with partnership of 160 registered nurses were chosen by census and 504 hospitalized patients were selected by quota sampling from medical and surgical wards affiliated to training hospitals of the Medical Sciences Jahrom University. Study tool was a questionnaire list, 31 questions for nurses and 22 questions for patients. Data analysis performed using, SPSS 16, connected Chi-Square, correlation and Mann-Whitney tests. Results: According to the nurses' perspective 72% had responsible aspect and respectful patients care quality performance was 70%. However from patient’s perspective 40% of nurses had responsible aspect, quality performance 45% and patients respect 69% to meet ethical standard. There was significant difference between nurses and patients perspective in sight of responsibility and quality care performance. Conclusion: The results showed that there is difference between patients and nurses' perspective about professional ethics standards compliance. Objective: The purpose of this study was to evaluate staff development program through west Mazandaran hospital nurses and its relationship with the level of their professional competency. Methods: This study was descriptive - correlations in 205 nurses from general ward who were selected by cluster sampling. Data was collected by two questionnaires for competency level and staff development programs. Face and content validity was approved and reliability of the instrument was assessed by the internal consistency. Analysis of variance and chi-square tests were used to analyze data. rResults: The majority of samples reported their competent level as ideal (75/6%), and the effectiveness of the program estimated as average (80/5%). variance analysis of data showed there are no significant difference in the mean scores of survey instrument staff development, among the nurses with varying levels of competence (p =0/4, df =2). Conclusion: Organizations spend enormous amounts of money to improve development programs. If these programs loss their effectiveness, it is time consuming and will damage the economy. This surgery has various complications. Electrolyte disorders are among common important complications among these patients. Objective: purpose of this study was to determine the level of electrolyte disorders and related factors in patients  after coronary artery bypass grafting  surgery hospitalized in cardiac care units. Methods: In this descriptive cross–sectional study, 288 Coronary Artery By Pass Graft (CABG) patients admitted to educational hospital were selected by gradual sampling. Data were collected by a three part researcher made questionnaire covering  demographics,  past medical history and levels of sodium and potassium through patients’ medical records. Statistical analyses were performed using appropriate tests (chi square test, fisher exact test and logistic regression). Rیافته ها: نتایج نشان داد که سبک فرزندپروری مقتدرانه هم با نمره کل مشکلات رفتاری درونی سازی (003/RResults: Findings showed that 16.7% of patients undergoing coronary artery bypass grafting were hyponatremia, 4.9% hypernatremia, 14.2% hypokalemia and 3.8% hyperkalemia. The results also indicated that there was a significant relationship between the factors such as body mass index, history of drug use, the number of vessels involved, the number of grafts, the length of time connected to mechanical ventilation, duration of artificial heart-lung machine use, aortic clamping time, and amount of intraoperative hypothermia, hemoglobin and hematocrit and postoperative electrolyte disorders (P<0.05). According to Logestic regression model, these factors were not associated to electrolyte abnormalities. Objective: This study aimed to determine the effect of acupressure on sixth spleen point (SP6) on relief of primary dysmenorrhea. Methods: This study is a clinical trial. Students with primary dysmenorrheal living in dormitory were studied during three menstrual cycles. Pain intensity was measured on the first cycle without intervention. Subjects were divided into two parallel groups (pressure on SP6 and placebo groups), using a randomized block design with odds ratio of 1:1 based on the intensity of pain. In the second and third cycles, pressure was exerted by the samples. Pain intensity was compared with the visual analogue scale before and after intervention. Samples, person dividing the groups, data analyzer were blinded to study groups. At the end data related to 30 students in experiment group and 32 students in control group were analyzed. T test, chi-square, Friedman, Mann-Whitney and ordinal regression were used to compare the pain severity in experiment and control groups. rResults: Pain severity up to 3 hours after giving pressure on SP6 (p<0.004) and four hours after giving pressure on control point (p<0.001) was decreased compared to before intervention. There was no difference observed pain in groups before and after intervention. They have responsibility for their own personal life too. Due to multiplicity of nurse’s roles they suffer under several psychological and physical stresses. Understanding the factors related to their employment such as shift work can improve validity and efficiency of this force which is important in human. Objective: This study aimed to determine the role of shift work and predictor variables on marital satisfaction of employed nurses through educational – medical centers. Methods: In this cross – sectional study, 500 marital employed nurses from 8 training medical centers in Rasht were selected using stratified random sampling. Information was collected using ENRICH marital satisfaction questionnaire for demographic data, social and shift work. For data analysis spss16, descriptive and inferential statistical tests (linear regression, Pearson's test, Regression Logistic Ordinal Model) were applied. rResults: Results showed that there is a significant relationship between marital satisfaction and shift work (P=0/003), history job (P=0/0001), personal income (P=0/002), the place of service (P=0/0001), employment status (P=0/0001), age (P=0/002). The findings showed each child can increase 1.65 times chance of lower satisfaction. Psychiatric ward was only ward related to marital satisfaction (p<0.0001). But there was not significant relationship between marital satisfaction job degree, sex and educational level. Conclusion: According to this findings, shift work, and some occupational factors (history job, income, place of service, employment status) are related to marital satisfaction for nurses. An aspect that has impact on their health is occupation and job satisfaction. Objective: Present study is designed to examine predictive factors of public health based on job satisfaction of health service centers' employees. Methods: This study is a cross sectional descriptive-analytic study conducted in 2012. In this study, census method was used for sampling. 252 personnel of health centers participated in this study and data collection tool included three questionnaires of Job Description Index (JDI), General Health Questionnaire (GHQ) and demographic characteristic questionnaire which were used after establishment of its validity and reliability. Data was analyzed using descriptive and analytical tests (Mann Whitney- Kruskal Wallis. Rیافته ها: نتایج نشان داد که سبک فرزندپروری مقتدرانه هم با نمره کل مشکلات رفتاری درونی سازی (003/RResults: Findings showed that only 47.6% of staff had good general health. About 63.16% of employees were satisfied most level of satisfaction was in colleague domain (76.83%) and least level of satisfaction was in salary domain (41.5%). Results indicated that from domain studied, only related and predicted areas of public health were colleague domain (p<0.049) and work environment condition (p<0.043) therefore the chance of general health increased by increasing level of satisfaction with colleague and work condition. Among the demographic characteristics, education (p<0.043) and management position (p<0.035) were identified as predictors of public health. Conclusion: In attention to obtained results, it can be said that the relationship between work and health is mutual and for improving the two concepts, planning and appropriate interventions are needed.
