<p dir="RTL"><strong>مقدمه: </strong><strong>پیامدهای</strong> <strong>روانی</strong><strong><span dir="LTR"> - </span></strong><strong>اجتماعی</strong> <strong>ابتلای به سندروم نقص ایمنی اکتسابی ممکن</strong> <strong>است</strong> <strong>مشکلات</strong> <strong>بیشتری</strong> <strong>از</strong> <strong>خود</strong> <strong>بیماری</strong> <strong>برای</strong> <strong>افراد </strong><strong>مبتلای به آن</strong> <strong>ایجاد</strong> <strong>کند. یکی از مهم­ترین پیامدهای</strong> <strong>روانی</strong><strong><span dir="LTR"> - </span></strong><strong>اجتماعی</strong> <strong>استیگمای اجتماعی ادراک­شده</strong><strong> در افراد با علائم سندروم نقص ایمنی اکتسابی</strong><strong>، </strong><strong>کاهش عزت نفس در بین این بیماران است، زیرا این</strong> <strong>بیماری</strong> <strong>با</strong> <strong>ایجاد</strong> <strong>تغییرات</strong> <strong>شناختی،</strong> <strong>روحی و</strong> <strong>روانی،</strong> <strong>تأثیر</strong> <strong>منفی</strong> <strong>زیادی</strong> <strong>بر عزت</strong> <strong>نفس</strong> <strong>بیماران </strong><strong>به دنبال دارد</strong><strong>.</strong></p>
<p dir="RTL"><strong>هدف: این</strong> <strong>مطالعه</strong> <strong>با</strong> <strong>هدف</strong> <strong>تعیین تأثیر استیگمای اجتماعی ادراک­شده در کاهش عزت نفس بیماران با علائم </strong><strong>سندروم نقص ایمنی اکتسابی</strong> <strong>انجام</strong> <strong>شد</strong><strong><span dir="LTR">.</span></strong></p>
<p dir="RTL"><strong>روش کار: مطالعه حاضر در قالب یک طرح همبستگی و به صورت مقطعی انجام شد. داده­های این مطالعه، از بین 63 بیمار با علائم ایدز جمع­آوری شد که جهت مشاوره و مداوا به مرکز مشاوره بیماری­های رفتاری یزد در سال 1391 مراجعه کرده بودند.</strong> <strong>این</strong><strong> بیماران</strong> <strong>با</strong> <strong>استفاده از </strong><strong>روش</strong> <strong>نمونه</strong><strong>­</strong><strong>گیری</strong> <strong>در</strong> <strong>دسترس</strong> <strong>انتخاب</strong> <strong>گردیدند</strong><strong><span dir="LTR">.</span></strong><strong> برای تعیین حجم نمونه، از فرمول اصلاح شده کاکران استفاده شد. ابزار گردآوری داده­ها عبارت بودند از: </strong><strong>مقیاس عزت</strong> <strong>نفس</strong> <strong>روزنبرگ </strong><span dir="LTR">(RSES)</span> <strong>و پرسشنامه اصلاح­شده استیگما در بیماران مبتلا به </strong><strong>سندروم نقص ایمنی اکتسابی. برای محاسبه پایایی ابزار اندازه­گیری، از روش آلفای کرونباخ استفاده گردید. </strong><strong>داده­ها با استفاده از آزمون مرتبه صفر پیرسون و رگرسیون چندگانه سلسله مراتبی تحلیل شدند.</strong></p>
<p dir="RTL"><strong>نتایج: </strong><strong>شصت و سه نفر (48 مرد و 15 زن) از بیماران با علائم سندروم نقص ایمنی مورد بررسی قرار گرفتند. دامنه</strong> <strong>سنی</strong> <strong>آزمودنی­ها</strong> <strong>بین</strong> <strong>14</strong> <strong>تا 56 سال، با میانگین 77/7 </strong><strong>±</strong><strong> 36 سال بود</strong><strong><span dir="LTR">.</span></strong><strong> نحوه ابتلاء به بیماری در بین پاسخگویان به ترتیب اعتیاد تزریقی 33 نفر (4/52%)، آمیزش جنسی 17 نفر (27</strong><strong>٪</strong><strong>)، نامشخص 9 نفر (3/14%)، خون و فرآورده های خونی 3 نفر (8/4%) و انتقال از مادر به جنین 1 نفر (6/1%) بوده است. طول</strong> <strong>مدت</strong> <strong>ابتلاء به ایدز بین 1 تا 15 سال با میانگین 85/7 </strong><strong>±</strong><strong> 15/3 بود. تفاوت معناداری میان مردان و زنان با علائم نقص ایمنی از نظر استیگمای اجتماعی ادراک­شده وجود نداشت. شیوه ابتلای به ویروس </strong><span dir="LTR">HIV</span> <strong>نیز تفاوتی در میزان استیگمای اجتماعی ادراک شده بیماران ایجاد نکرده بود. </strong><strong>همبستگی معنادار معکوسی بین استیگمای اجتماعی ادراک­شده با متغیر عزت نفس </strong><strong>(</strong><strong>01/0> </strong><strong><span dir="LTR">P</span></strong><strong>) و مولفه پذیرش خود </strong><strong>(</strong><strong>01/0> </strong><strong><span dir="LTR">P</span></strong><strong>) وجود داشت. همچنین، همبستگی معکوس معناداری بین مولفه­های استیگمای اجتماعی ادراک شده، یعنی استیگمای شخصی با عزت نفس </strong><strong>(</strong><strong>05/0> </strong><strong><span dir="LTR">P</span></strong><strong>)، و خودانگاره منفی با عزت نفس </strong><strong>(</strong><strong>01/0> </strong><strong><span dir="LTR">P</span></strong><strong>) وجود داشت. </strong><strong>تحلیل</strong> <strong>رگرسیون</strong> <strong>چندگانه</strong> <strong>سلسله</strong> <strong>مراتبی</strong> <strong>نشان</strong> <strong>داد که</strong> <strong>متغیرهای دسته اول (اجتماعی-جمعیت شناختی و ویژگی های بالینی بیمار) که در مرحله اول وارد شدند، سهمی در تبیین واریانس متغیر عزت نفس نداشتند. در مرحله دوم، و با ورود متغیر استیگمای اجتماعی ادراک­شده در مدل، این متغیر توانست </strong><strong>4/23</strong> <strong>درصد (234/0 =</strong><span dir="LTR">R</span><strong><sup><span dir="LTR">2</span></sup></strong><span dir="LTR"> Adj</span><strong>) از</strong> <strong>واریانس</strong> <strong>عزت نفس</strong><strong> را</strong> <strong>تبیین</strong> <strong>نماید.</strong></p>
<p><strong><span dir="RTL">بحث و </span><span dir="RTL">نتیجه­گیری: </span><span dir="RTL">استیگمای اجتماعی ادراک­شده یکی از عوامل خطر عمده، نارسایی عزت نفس در بیماران با علائم </span><span dir="RTL">سندروم نقص ایمنی اکتسابی است</span><span dir="RTL">. بنابراین، </span><span dir="RTL">با</span> <span dir="RTL">ارائه</span> <span dir="RTL">مداخلات</span> <span dir="RTL">رفتاری ـ اجتماعی</span> <span dir="RTL">مناسب</span> <span dir="RTL">به</span> <span dir="RTL">بیماران،</span> <span dir="RTL">خانواده­ها و مردم</span> <span dir="RTL">در</span> <span dir="RTL">جهت اصلاح باورها نسبت به اختلال نقص ایمنی، می­توان به بهبود عزت نفس این بیماران کمک کرد</span></strong></p>
<p><strong>Introduction:</strong> Psychosocial consequences of Humanity Immunity Virus infection may create more problems than disease itself. One of the psychosocial consequences of perceived social stigma in patients with acquired immunodeficiency syndrome is decrease of self-esteem, because this disease has numerous negative effects on self-esteem of patients through cognitive and psychological changes.</p>
<p><strong>Objectives</strong>: The aim of this study was to determine the relationship between perceived social stigma and self-esteem among patients with AIDS.</p>
<p><strong>Methods:</strong> This study was conducted with correlation and cross-sectional design. Data were collected from 63 patients with HIV symptoms who referred to Yazd behavioral disease clinic in 2012. These patients were chosen by convenience sampling method. Sample size was determined using Cochran formula. Measuring tools included Rosenberg Self-esteem Scale and Perceived Stigma Questionnaire and reliability of tools was calculated using Alpha Cronbach. Data were analyzed using zero-order Pearson correlation, independent t-test, one-way analysis of variance (ANOVA), and the hierarchical multiple regression methods.</p>
<p><strong>Results:</strong> Sixty-three patients (48 men and 15 women) with symptoms of immune deficiency syndrome were studied. Participants aged between 14 and 56 years with an average of 36±7.77 years. Mode and number of HIV infection included: drug injection 33 (52.4 percent), sexual intercourse, 17 (27%), unknown 9 (14.3%), blood and blood products 3 (4.8%) and transmission from mother to fetus 1 patients (1.6%). Also, the duration of HIV infection was between 1 and 15 years with an average of 3.15 ± 7.85. There was no significant difference between men and women with symptoms of immunodeficiency in terms of perceived social stigma. Also, way of contracting the HIV virus did not make a difference in patients' perceived stigma. There was reversed significant correlation between perceived social stigma and self-esteem (P<0.01) and component self – acceptance (p<0.01). Also, there was reversed significant correlation between components of perceived stigma, personal stigma and self-esteem (P<0.05), and negative self-image and self-esteem (P<0.01). Hierarchical multiple regression analysis showed that the first group of variables (socio-demographic and clinical characteristics of patients) included in the first stage, did not share in explaining the variance in self-esteem. In the second stage, and with the arrival of perceived social stigma variables in the model, this variable could explain for 23.4% of variance in self-esteem (Adj. R<sup>2</sup> = 0.234).</p>
<p><strong>Conclusion: </strong>Perceived social stigma acts as a risk factor for decease of self-esteem in patients with HIV/AIDS. Therefore, presenting proper socio-cultural interventions to these patients, families and people to correct their beliefs regarding HIV can help promote patients’ self-esteems</p>
