تعیین وضعیت حرکتی کودکان مبتلا به اختلالات مهارتهای حرکتی با استفاده از دو مقیاس بازتاب و هوشبهر حرکتی

<p><span style="font-weight: bold"> مقدمه: </span>تعیین پیش آگهی کودکان مبتلا به اختلالات حرکتی موضوع مهمی برای والدین و متخصصان می باشد. پیش بینی وضعیت رشدی کودک در سنین پائین بسیار مشکل است. از آزمون های در دسترس و ساده که بتوان تاحدی وضعیت آتی را پیش بینی نمود، بررسی بازتابهای حرکتی Bleck و هوشبهر حرکتی Capute را می توان ذکر کرد.<br>
<span style="font-weight: bold">هدف:</span> این مطالعه با هدف تعیین ارتباط بین دو آزمون Capute و Bleck در تعیین وضعیت حرکتی کودکان مبتلا به اختلال مهارتهای حرکتی انجام شده است.<br>
<span style="font-weight: bold">روش کار: </span>در این مطالعه مقطعی تعداد 40 کودک پایین تر از 2 سال به صورت در دسترس از میان کلینیک های کاردرمانی دانشگاه علوم پزشکی تهران طی سال های1389تا1390 انتخاب شدند. ارزیابی بازتابهای حرکتی اولیه نوزادی بر اساس مقیاس Bleck و هوشبهر حرکتی بر اساس مقیاس Capute انجام گردید. داده ها با استفاده از آزمون های تی تست، ضریب همبستگی پیرسون، کای اسکوئر و کروسکال والیس ارزیابی گردید.<br>
<span style="font-weight: bold">نتایج: </span>در این مطالعه 40 کودک(15دختر و 25پسر) با میانگین سنی43/5±43/15 ماه شرکت کردند. میانگین نمره هوشبهر حرکتی Capute 29/19±80/38 بود و 7/7درصد از کودکان پیش آگهی خوب، 4/15درصد پیش آگهی متوسط و 9/76درصد پیش آگهی بد داشتند. بر اساس نمره بازتاب حرکتی B‏leck 5/17درصد از کودکان در طبقه بندی پیش آگهی خوب، 5/42درصد پیش آگهی متوسط و 40درصد در گروه پیش آگهی بد قرار گرفتند. بین نمرات بازتابهای حرکتی Bleckو نمرات هوشبهر حرکتی Caputeارتباط معنی داری مشاهده نشد.<br>
<span style="font-weight: bold">نتیجهگیری: </span>با وجود اینکه هر کدام از بازتابهای نوزادی بر مراحل رشد حرکتی و نهایتاً تعیین پیش آگهی حرکتی تأثیرگذارند، ولی ارتباط معنی داری بین بازتابهای حرکتی و هوشبهر حرکتی مشاهده نشد. بنابراین بنظر می رسد این ابزارها به تنهایی قادر به پیش بینی وضعیت حرکتی آتی کودک نیستند و باید در کنار هم بکار گرفته شوند.</p>
<p><span style="font-weight: bold">Introduction:</span> The motor prognosis of children with motor skills disorders is a major issue for their parents and the health professionals involved in their management. Prediction of prognosis in infancy is very difficult. Available and simple tests which can examine prognosis are Bleck infant’s reflexes and motor quotient of Capute.<br>
<span style="font-weight: bold">Objective:</span> The aim of this study was to investigate the relationship between Bleck and Capute tests for determination of motor status of children with motor skills disorders. Methods: In this cross - sectional study, the sample consisted of 40 infants under 2 years of age with motor disorders who referred to occupational therapy clinics of Tehran University of Medical Sciences from 2011 to 2012. The infants were evaluated with Bleck and Capute tests. In Bleck scale, primitive reflexes were evaluated. Capute scale is based on motor quotient. The relationship between these tests was examined using Chi-Square and Kruscal Wallis tests.<br>
<span style="font-weight: bold">Results:</span> In this study 40 infant (15 girls and 25 boys) with mean age of 15.43 months participated. Mean score of Capute scale was 38.80 ± 19.29 and 7.7% of infants were classified as good prognosis, 15.4% as guarded prognosis and 76.9% in poor prognosis. Based on Bleck Scale 17.5% of infants were in good prognosis category, 42.5% in guarded prognosis and 40% in poor prognosis. There was no significant correlation between Bleck and Capute tests.<br>
<span style="font-weight: bold">Conclusion:</span> Although infant reflexes affect motor milestones and eventually determination of motor prognosis, but in this study there was no significant relationship between Bleck infant’s reflexes and motor quotient of Capute. Therefore it seems that these scales are not alone capable of predicting child's motor status and should be used together.</p>
